Title from inside lid of box, and from laid-in colophon ([7] p.). Colophon includes directions for folding the book for placement in the box. Twenty-one numbered copies and two artists' proofs, signed by Daniel E. Kelm, Erin Clay Nelson, Sammy Lee, Anthony J. Fiandaca, Amy Borczo, and Kylin Lee. Originally published by Kelm in an edition of 5 copies in 1990 (distributed by Granary Books).
